dc.description.abstractPaństwowe Ratownictwo Medyczne (PRM) jest ważnym elementem systemu ochrony zdrowia i bezpieczeństwa publicznego. Celem rozprawy jest analiza funkcjonowania PRM w Polsce oraz identyfikacja rozwiązań zwiększających jego efektywność operacyjną i systemową. Praca opiera się na założeniu, że dotychczasowe reformy PRM miały głównie charakter reaktywny, co ograniczało ich skuteczność wobec wyzwań społecznych, demograficznych i organizacyjnych. Hipoteza badawcza zakłada, że wdrożenie odpowiednich rozwiązań organizacyjnych, technologicznych i prawnych może zwiększyć efektywność systemu. Analizie poddano m.in. rolę wolontariatu, kompetencje ratowników medycznych, strukturę zespołów ratowniczych, bezpieczeństwo farmakoterapii pediatrycznej oraz procedury związane ze stwierdzaniem zgonu. Rozprawa została osadzona w paradygmacie nauk o polityce, traktując PRM jako element polityki publicznej i systemu bezpieczeństwa państwa. Obejmuje analizę uwarunkowań prawnych i organizacyjnych oraz badanie ankietowe przeprowadzone wśród pracowników systemu. Wyniki wskazują, że system realizuje podstawowe zadania operacyjne, jednak zmaga się z ograniczeniami strukturalnymi. Główne problemy to przeciążenie zgłoszeniami, niejednoznaczności prawne i organizacyjne, wydłużające się czasy reakcji oraz ograniczona gotowość do reagowania na zdarzenia masowe. Wyniki potwierdzają potrzebę dalszych reform. The State Medical Rescue System (PRM) is an important component of healthcare and public safety. The aim of this dissertation is to analyse the functioning of PRM in Poland and identify solutions that may enhance its operational and systemic effectiveness. The study is based on the assumption that previous PRM reforms have been largely reactive, limiting their effectiveness in addressing social, demographic, and organisational challenges. The main research hypothesis assumes that appropriate organisational, technological, and legal solutions may improve system performance. The analysis covers the role of volunteering, paramedic competencies, the structure of emergency medical teams, the safety of paediatric pharmacotherapy, and legal conditions affecting their work. The dissertation is grounded in the political science paradigm, treating PRM as an element of public policy and the state security system. It includes an analysis of legal and organisational determinants and a survey conducted among system personnel. The findings indicate that the system performs its basic operational tasks but faces structural limitations. The main challenges include overload, legal and organisational ambiguities, increasing response times, and limited preparedness for mass-casualty incidents. The results confirm the need for further reforms
